Community Care Teams Being Introduced

Community Care Teams help to close the gap between people with chronic health needs and their home care. By giving them a team of providers, the CCTs are helping to improve their care. They are also reducing costs for citizens by keeping them healthy at home.

This, in turn, will help to avoid hospital admissions and ER visits. It will also ensure that they are able to stay home instead of being re-admitted to the hospital.

This type of care system has worked well in several states. North Carolina and Vermont have benefited already from this model. Additionally, New York residents have access to CCTs.

Medicare has allowed for this initiative to now be possible in Maine. There are already eight providers who offer Community Care Teams. This should help to close the gap between people who need services and those that find there is a barrier to going to the doctor.
