Clinical Improvement at Maine Quality Counts (QC) leverages quality improvement science to better support primary care practices and health systems in improving population health outcomes. Quality improvement science is a systematic approach for identifying problems and planning change, while also testing those plans and sustaining change. We provide education, peer support, and expert consultation designed to help healthcare providers increase their knowledge and self-efficacy in evidence-based standards of patient care.
Practice facilitators at QC provide practice-level coaching and support, teaching care teams the skills and tools they need to implement standardized, systemic, and team-based processes that will improve patient care. Our goal is to empower practices to implement workflows that also simultaneously improve the patient experience.
